Hoarding Disorder:
Because of the distinctiveness of hoarding symptoms, the DSM-V diagnostic work group on OCD has recommended that hoarding be included in DSM-V. However, the committee is still examining the evidence to determine whether to include “hoarding disorder” in the main part of the manual or in an appendix for further research. The proposed diagnostic criteria are:

A. Persistent difficulty discarding or parting with personal possessions, even those of apparently useless or limited value, due to strong urges to save items, distress, and/or indecision associated with discarding.

B. The symptoms result in the accumulation of a large number of possessions that fill up and clutter the active living areas of the home, workplace, or other personal surroundings (e.g., office, vehicle, yard) and prevent normal use of the space. If all living areas are uncluttered, it is only because of others’ efforts (e.g., family members, authorities) to keep these areas free of possessions.

C. The symptoms cause clinically significant distress or impairment in social, occupational, or other important areas of functioning (including maintaining a safe environment for self and others).

D. The hoarding symptoms are not due to a general medical condition (e.g., brain injury, cerebrovascular disease).

E. The hoarding symptoms are not restricted to the symptoms of another mental disorder (e.g., hoarding due to obsessions in Obsessive-Compulsive Disorder, lack of motivation in Major Depressive Disorder, delusions in Schizophrenia or another Psychotic Disorder, cognitive deficits in Dementia, restricted interests in Autistic Disorder, food storing in Prader-Willi Syndrome).

Individual Treatment for Hoarding

Hoarding is a difficult problem to treat. Neither medicine nor psychological therapies that work well for OCD seem to be as effective for hoarding. A special form of cognitive behavior therapy has been developed to treat hoarding. This method combines:
  • elements of motivational interviewing
  • several features of cognitive therapy and behavioral practice for OCD, and
  • skills training
The treatment focuses on three hoarding behaviors:
  • excessive acquisition
  • difficulty discarding or letting go of possessions, and
  • disorganization and clutter that impairs functioning.

2016 01 19 01.
New computer. It's 4:20. I'm sitting in the lobby of the clinic, waiting till, I hope 5 pm, when they open the doors to our clinic. I can't get online and I don't feel much like working on the brief. Walked here through falling snow. I bit down. What I got done today: 1. fixed my other computer for $20, by buying a battery and a power cord.  2. emails to my lawyer's office. 3. a load of laundry. 4. caught 2 buses to get back to the clinic. so 4 things, not too bad. maybe i can do one or two more things.
to do: scrub social media of any mention of the accident. call 4015 ask if they got wifi hooked up. suggest they get rat poison, rat traps, get reciepts, i'll pay for it when i get back. mouse poison too. $20 or so.
did not sleep well last night either. they get us up at 5, keep the bunk rooms locked, people snore or make calls. i should start going to sleep after dinner.
Wow really nice interaction with mohammed. he got me online, got me water,
we talked about the different languages he knows. a nurse, i know her name, came by and we talked shop about my new computer, because she remembered my pink one; this is the same model in blue. she says, and she's right, that it doesn't have enough internal memory, 2 gigs, so i'll need to remember not to save pictures except online, and no movies and such. Parriss, I think that's her name. Which ties in with Mohammed, who speaks French when he is home in Toronto, the home office of this company.
He also speaks arabic and some spanish because he's from Morocco. He doesn't look like a Moor but could pass as Spanish. English and French and Arabic and Spanish, I might be forgetting one. I dont know if he's a doctor as well as being a tech guy, maybe just IT but probably more than that.
So that was a fun adventure all in the past 15 minutes. I'm online so  I could make a comic. Plan for tonight: check-in, shower, nap, work on brief. See if other computer powers up ok, email myself files. Ah, this would be a good time to try again to download openoffice.

mom needs a hearing aid where she doesnt have to keep changing the batteries - that's her current objection to getting a hearing aid. i dont know anything about these, but i did recently learn that the newer hearing aids are rechargable. so in this entry i'll put a few links.

from what i'm seeing so far the $300 seimens charger looks like the way to go. you just set in its cradle at night and it recharges. not a cost savings, but more convenient if you dont mind taking it in and out each night.
alternative is have Zach change the batteries every couple weeks as needed.

my computer is dying. so i'll try writing by hand. i'm on a bus. i di a loop back to n hanley (a train station in the Ferguson ghetto area) because i misswed my stop and the orange bus doesn't start till 9 anyway. I'm a bit concerned over wehre do i pick up this bus tomorrow. -the orange line stops right at pharmamedica - to go back to the st louis greyhound.
i am just assuming they will send me home. so maybe illegible is where to wait for the shuttle "SCAT" in the morning. Foundry Art center. outside the hostpital inthe cold is the other option, the bus comes once every hour and 15  minutes and can be 15 minutes late.

talk to sukie about doing that as a grant proposal. something like a clinic possibly working out of the worker justice center that would use law students to assist unrepresented persons in H + H courts and try to  engage the legal  community into doing some pro bono; set it up as a CLE with participation option. involve MCBA.
